What We Offer
• Specialist Support Coordination
For participants with high or complex needs, we offer additional guidance around:
- Placement transitions (out of hospital, out-of-home care, or crisis settings)
- Navigating restrictive practices and safeguarding needs
- Advocacy during service breakdowns or plan reviews
- Behaviour support integration and multi-stakeholder case management
• Support Coordination (Level 2)
Our team works closely with families to:
- Unpack your NDIS plan and budget
- Identify and connect with therapists, care providers, and community services
- Coordinate care around your life—not the other way around
- Review service agreements, book supports, and problem-solve gaps in care
- Prepare for plan reviews with strong reports and outcome tracking
